• Parent's Consent Form for the City
• Medical Release Form� This form should be kept with the coach/team for emergency medical suituations.� It is required at tournaments.� Do not turn in a copy to the City or County.� CGSA would like a copy on file at the office.

Georgia Youth Soccer Rules
GYSA mandates that all players shall play at least half of a game. Click here for GYSA rules.� Section 500 deals with the modified rules for youth play and includes rules regarding substitution and playing time.
